Pain in the hip, buttock or down the back of the leg is one of the most misunderstood problems we see. The first job is working out what is actually causing it, because muscular referral and genuine nerve irritation need different treatment.

Understanding hip and sciatic pain.
The word sciatica gets used for almost any leg pain, but the causes vary widely. Sometimes it is true irritation of the sciatic nerve or the nerve roots in the lower back. Often it is referral from the gluteal muscles, the deep hip rotators or the muscles around the pelvis, which can produce pain in exactly the same places. Hip pain itself may come from the joint, the tendons on the outside of the hip, or the muscles that control the pelvis while you walk and stand on one leg.
Treating the driver, not just the sore spot.
Assessment comes first and it matters more here than almost anywhere else. We test nerve sensitivity, hip joint range and the strength and control of the muscles around the pelvis, then treat according to what we find. Where the driver is muscular, hands on release of the glutes and deep rotators, plus dry needling into the trigger points that are referring, usually produces quick change. Where there is genuine nerve involvement, treatment is gentler and focuses on reducing sensitivity and giving the nerve room, with graded movement rather than aggressive stretching.

What treatment looks like.
Expect a longer conversation and more testing in your first visit than you might be used to. From there, hip pain driven by soft tissue often responds within a few sessions. Nerve related pain tends to be slower and less linear, with good days and bad days, so we set realistic expectations early. In both cases the plan includes strengthening for the glutes and hip control, because that is what stops the same pattern returning once the pain has quietened down.
We start with a proper conversation about your history, how the pain behaves through the week and what you want to get back to, followed by hands on and movement assessment.
